How do I access my patient portal?
My new electronic medical records system and patient portal is called Athena. I will be losing access to MyChart in the coming months. In order to access Athena, please submit your name and email through this website.
Where is your office?
My office address is 2845 N Sheridan Suite #910, Chicago, IL 60657.
NOTE: I will be using a temporary suite - 907- until my permanent suite is ready. Suites 907 and 910 are right next to each other.
